This Offer is addressed to an indefinite number of individuals and legal entities – visitors of the website on the Internet: https://www.cmaukraine.com (hereinafter referred to as the "Website"), hereinafter referred to as the "Donor", and is an official and public proposal of the Charitable Organization “Charitable Fund ‘CHRISTIAN MEDICAL ASSOCIATION OF UKRAINE’” (registration number 44855996), hereinafter referred to as the "Fund", represented by Director Regina Yuriyivna Maistrova, acting based on the Charter, to enter into a voluntary charitable donation agreement (hereinafter referred to as the "Agreement"), the subject and essential terms of which are specified below:

1. Terms and Definitions:

Acceptance: Full and unconditional acceptance of the Public Offer by performing actions to make a money transfer using payment forms and tools available on the Website, as well as by transferring funds to the Fund's bank account through banking institutions. The Acceptance Date is the date of crediting the funds to the Fund's account.

Donation: Voluntary charitable donation.

2. Subject of the Agreement:

The subject of this Agreement is the gratuitous transfer of funds by the Donor to the Fund for the purpose of achieving the goals and charitable activities of the Fund.

Acceptance of the Agreement means that the Donor agrees with all its provisions, fully understands the subject of the Agreement, and the purpose of collecting donations.

The Agreement is concluded by the unconditional and full accession of the Donor to this Agreement and acceptance of all its essential terms.

The Donor and the Fund, guided by Article 207, Part 2 of Article 639, Articles 641, 642 of the Civil Code of Ukraine, and Article 7 of the Law of Ukraine "On Charitable Activities and Charitable Organizations", agree that the Agreement is considered concluded in writing without signing a written copy by the Parties from the moment the Donor performs the actions provided for by the Agreement, indicating agreement to comply with its terms.

The Agreement is posted on the Website https://www.cmaukraine.com in open access and in a manner that ensures every person contacting the Fund can familiarize themselves with its content.

The Donor cannot propose their own terms for the Agreement.

3. Rights and Obligations of the Parties:

The Fund has the right to:

Receive voluntary charitable donations and use them to achieve the goals and charitable activities of the Fund in accordance with the Agreement and the Law of Ukraine "On Charitable Activities and Charitable Organizations".

At the Donor's request, provide a report on the received voluntary charitable donation and its use.

The Donor has the right to:

Transfer a voluntary charitable donation to the Fund's account in the manner specified in the Agreement. Request a report on the use of the voluntary charitable donation.

4. Place of Fundraising:

The collection of voluntary charitable donations is carried out worldwide.

5. Term of Fundraising:

The collection of voluntary charitable donations continues until the Fund's liquidation unless another term is determined by the Fund.

6. Procedure for Public Access to the Fund's Reports:

Access to the Fund's reports is provided by posting them on the Website in the "PROJECTS" section. Other information may be provided by the Fund in the manner and terms provided by the legislation of Ukraine.

7. Payment of Fees:

All expenses related to the payment of amounts associated with the transfer of donations are borne by the Donor.

8. Other Conditions:

The Donor independently determines the amount of the donation. The donation is voluntary and non-refundable. The Donor is responsible for the accuracy of the information provided when making the Donation.

For the purpose of complying with the Law of Ukraine "On the Protection of Personal Data" and properly fulfilling the terms of the Agreement, the Donor.
